A board decision record should show what changed.

This specimen shows the form of the written record delivered after a Board Session. It is not a client case, testimonial or claimed commercial outcome.

Decision context.

A board is considering AI across contracts, leases, reports and operational records. It needs to decide where to begin, what evidence is required before capital is committed, and which controls are non-negotiable.

Direction

Begin with one document-heavy workflow where the current decision, delay or error can be observed. Do not begin with an enterprise platform purchase.

Capital

Approve a fixed-scope Proof only. No production Build is authorised until the workflow meets written acceptance criteria on the organisation’s own material.

Risk controls

No unsourced answer may be relied on. Approved data boundaries, named human review, uncertainty reporting and an audit log are required from the first test.

Ownership

The executive sponsor owns the decision. A workflow owner supplies the material and signs the acceptance report. Technology supports the environment; it does not own the business result.
